Student Application and Recommendation Forms

Upon completing both application forms, you can do one of the following:

The following information provides a general set of guidelines for applicants as well as an outline of the criteria the Board of Trustees will be looking for in selecting scholarship recipients.

Application Guidelines

Applicant must be between the ages of 17 and 23.
Applicant must be living in out-of-home care or have been emancipated from out-of-home care due to state age limit. We understand that there are many unique situations. Generally, out-of-home care means living on your own or with a caregiver who is not an adoptive or natural parent. If you have questions about your eligibility, please contact us.
Applicant must be unmarried and without children.
Applicant must be either enrolled in, or accepted to an accredited post-secondary academic program.

Applicant must submit the following:

Foundation application
Two recommendation forms, one from a teacher and the other from a school counselor, care giver or foster care agency.
High school transcript and an SAT or ACT score (if applicable). If you are already attending a post-secondary program, then the previous year's transcript is to be submitted. If you are in your first semester, please submit high school transcript, SAT/ACT score and mid-term grade report (if available.) If no mid-term report is available, please submit transcript showing current course enrollment.
Copy of Student Aid Report (SAR) or current financial aid package if already attending college.
Applicant must also apply for financial aid through the college or university.
Applicant must participate in an interview by the Board of Trustees.

Selection Criteria

The financial need of the student
Academic performance
Contribution to the school community
Teacher and/or caregiver recommendations
The information provided on the student application form, including the short answer and essay
